POSITION SUMMARY
The HIV Prevention Navigator will be responsible for overseeing & managing HIV prevention services including, but not limited to, HIV education; HIV testing & counseling; PREP coordination, and linkage to care for HIV positive patients. The HIV Prevention Navigator will also be responsible for fostering & developing relationships with community partners (new & existing) to support the organizational goal of expanding HIV prevention services to high-risk, high-need communities.
MINIMUM QUALIFICATIONS
• Bachelor’s degree or equivalent experience required.
• At least 2-5 years working in HIV prevention and/or care required.
• Supervisory experience required.
• Must have experience working with diverse populations including patients with mental health and substance use disorders.
• Strong knowledge of HIV prevention & best practices.
• Must possess the ability to speak effectively before groups and actively engage the general public in outreach situations.
• Ability to effectively develop and nurture relationships with a diverse group of stakeholders.
• Must be well organized, self-starter with strong verbal, written, computer, and communication skills.
• Experience with outreach and organizing community/organizational events preferred.
• Data analytic skills a plus
• An automobile, a valid driver’s license and vehicle insurance coverage required.
RESPONSIBILITIES
- Management & oversight of HIV prevention services.
- Provides leadership & direct supervision to the HIV Outreach and Prevention Coordinator(s).
- Develop, discover and attend community events in order to promote HIV prevention services.
- Collaborate with various local organizations to build awareness of HIV prevention services, identify at risk populations, and build referral linkages.
- Design & maintain a workflow for PREP navigation services.
- Participate in key HIV committees and workgroups impacting HIV and related care in the Chicagoland area.
- Collaborate with care team to effectively identify at-risk patients.
- Monitor, track and evaluate program performance and provide summary reports to direct supervisor on a monthly basis and/or when requested.
- Develop and implement goals and objectives for the HIV Outreach and Prevention team that supports program and organizational objectives.
- Recommend personnel actions; assess staff performance; mentor/coach/train staff and provide direction and feedback; participate in team meetings; plan agendas for meetings; prepare technical reports.
- Act as the liaison to appropriate levels of leadership with various external audiences regarding the promotion and enhancement of HIV prevention services across the care continuum.
- Assist in the development and implementation of organizational outreach and HIV prevention initiatives.
- Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
- Develop and build relationships within the community and with other organizations to support HIV prevention initiatives.
- Ensure that all program scopes and criteria are met or exceeded.
- Provide unbiased information to consumers about HIV prevention, testing, and care.
- Conduct community outreach and engagement events including public events and business presentations.
- Coordinate and ensure alignment of the outreach activities/events with program requirements.
- Distribute educational materials to patients, community members, partner organizations and businesses to build HIV prevention awareness in high risk, high need communities.
- Conduct HIV prevention activities including testing, counseling, and patient follow up
- Support service integration efforts together with the ambulatory, behavioral health, oral health and social service teams.
- Serve as a resource to the care team and other staff members on HIV prevention services, including benefits of PREP.
- Perform other related duties as assigned.
