HIV Prevention Navigator
Under the guidance of the HIV Prevention Navigator, the HIV Outreach and Prevention Coordinator will be responsible for providing HIV prevention and outreach services to high risk, high need communities. This includes, but is not limited to, field outreach to identify at risk individuals; providing HIV education, testing & counseling; and engaging patients in primary care, HIV treatment, and/or PrEP. The HIV Outreach and Prevention Coordinator will also be responsible for providing care coordination services to patients receiving PrEP.
High school diploma or GED from an accredited school/program.
Must be a certified HIV/AIDS counselor or have the ability to complete training within 6 months of employment.
At least 2 years of experience working with individuals impacted by HIV/AIDS.
Must have experience working with diverse populations including LGBTQ communities and patients with mental health or substance use disorders.
Knowledge of HIV prevention & best practices highly preferred.
Previous community outreach experience.
Must possess the ability to speak effectively before groups and actively engage the general public in outreach situations.
Must be well organized, self-starter with strong verbal, written, computer, and communication skills.
Ability to work evenings and some weekends.
Ability to remain professional at all times when communicating with patients, staff members, community partners, external agencies, etc.
Knowledge of PrEP a plus.
Basic Microsoft office skills.
Prior health/medical and /or social services experience preferred.
Personal Vehicle & Vehicle Insurance required.
Provide individual and group level HIV prevention education, testing, and counseling.
Conduct outreach to link patients to HIV prevention, medical, and other supportive services (i.e. primary care; HIV treatment; mental health; PrEP).
Provide unbiased information to consumers about HIV prevention, testing, and care.
Conduct & participate in community outreach events including public events and health fairs to spread HIV prevention awareness.
Coordinate care for patients receiving PrEP to ensure patient adherence to medication and regularly scheduled visits.
Conduct adherence education with patients as needed.
Prepare HIV educational and informational resources for distribution at community outreach events and engagements.
Distribute educational materials to patients, community members, partner organizations and businesses to build HIV prevention awareness in high risk, high need communities.
Maintain appropriate documentation of services provided in patient medical charts, referral binder, etc.
Serve as a resource to the care team and other staff members on HIV prevention services, including benefits of PrEP.
Maintain working relationships with HIV Team members to enhance coordination of services.
Facilitate patient referral process including follow up and tracking.
Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
Implement and maintain department’s process for logging/tracking all patient outreach efforts made (i.e. events attended; appointment scheduling; patient no show follow up; etc.).
Network with HIV community to market and enhance program services.
Participate in HIV workgroups, networks, councils, or committees as requested.
Submit performance reports to direct supervisor as requested.
Participate in clinical and administrative huddles as assigned.
