POSITION SUMMARY
The Housing Program Supervisor oversees a team of case managers ensuring effective delivery and quality of case management services.
MINIMUM QUALIFICATIONS
- Bachelor’s degree required.
- A minimum of three years’ experience in management or social service coordination preferred.
- Experience serving special populations including but not limited to chronically homeless, individuals with mental illness, ex-offenders, individuals living with HIV/AIDS and/or substance abuse population is preferred.
- An automobile, valid Driver’s License and vehicle insurance coverage is required.
- Occasional late or weekend hours may be required.
RESPONSIBILITIES
- Provide supervision of case managers and monitor progress toward meeting program scopes/objectives.
- Conduct interviews and performance evaluations, recommends hires and disciplinary action.
- Conduct quality assurance activities including chart review and satisfaction surveys, to ensure 100% compliance with program scopes and eligibility.
- Conduct written monthly case management supervision, including the review of required case management documentation as outlined in the program procedures.
- Provide day to day supervision and oversight of direct reports.
- Oversee the process for timely, clean, and thorough data entry into HMIS and Client Track.
- Ensure the maintenance and organization of case management records/files.
- Monitors the intake and discharge processes • Review all initial assessments and ensure that accurate and timely assessments, re-certifications and service plans are completed.
- Schedule and oversee the facilitation of case management staffing.
- Facilitate group level staffing's or client consultations in collaboration with CCHC’s Behavioral Health department and or external community based resources.
- Periodically review charts to ensure appropriate documentation of participant program eligibility is maintained in all program charts.
- Review case management charts and case notes to ensure timely updates of service planning, follow-up, and case management tasks are aligned with service plan goals.
- Monitor the system of tracking, documenting and following up on referrals.
- Maintain schedules/itineraries for all team members home visits and reconciles mileage reimbursement requests with case notes and home visit itineraries.
- Responsible for the orientation and onboarding of new case managers.
- Review individual service plans and assess client progress notes completed by case managers.
- Routinely assesses strengths and barriers to increasing income, housing stability, employment, and increasing self-sufficiency (including health, substance use and mental health issues), and share resource referrals with team.
- Provides advocacy, support, and linkages/referrals to community resources.
- Monitors progress of group life-skill training with clients.

- Develops and maintains relationships with community agencies and landlords.
- Attends professional workshops and seminars to enhance skills and to maintain knowledge of policy changes.
- Attends monthly Housing Operations Team meetings and all other relevant program meetings as scheduled.
- Review and recommend changes to CCHC’s policies and procedures annually or as appropriate.
- Coordinate continuous case management trainings.
- Research, design, and implement program enhancements.
- Participate in quality initiatives of the organization and on external policy and quality committees at the COC or related level.
- Performs other duties as required or assigned.
