As a Personal Care Partner III, you will be an integral part of this team, working closely with our Health and Wellness Director to ensure the best possible outcomes for our residents.
Responsibilities:
- Work closely with the Health and Wellness Director to identify and address care needs.
- Conduct audits and reviews to ensure compliance with regulations.
- Provide guidance and support to QMAP care staff.
- Develop and implement care plans for residents.
- Communicate effectively with residents, family members, and healthcare professionals.
Qualifications:
- QMAP-A/L, Med Tech, Med Aide, or Nurse Delegation certification.
- At least one year of medication administration experience.
- High school diploma or equivalent.
- Effective communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Proficient in electronic health record management systems.
