About the Role
We are seeking a detail-oriented and passionate MDS Coordinator (RN) to join our skilled nursing community in Centennial, Colorado.
Key Responsibilities:
- Gather and assess resident data to develop personalized care plans.
- Collaborate with the care team to ensure each resident receives necessary support.
- Complete and submit MDS assessments on time to maintain compliance.
- Stay up-to-date with MDS regulations and best practices.
- Train and support staff to understand MDS processes.
- Promote quality assurance efforts by identifying areas for improvement.
- Communicate effectively with residents and families.
- Work with the billing team to align assessments with reimbursement requirements.
- Mentor new team members and guide them through the process.
Requirements:
- Current Registered Nurse (RN) license in the state of Colorado.
- Bachelor's Degree in Nursing (BSN) preferred.
- MDS Certification preferred.
- Minimum 2-3 years of experience as an RN in long-term care or skilled nursing community.
- Proven experience with MDS assessments and care planning.
- In-depth knowledge of Medicare and Medicaid regulations.
- Strong clinical assessment skills and attention to detail.
- Proficiency in electronic health record systems and MDS software.
- Excellent communication and interpersonal skills.
- Ability to manage time effectively and prioritize tasks.
- Thorough understanding of the Resident Assessment Instrument (RAI) process.
