Summary/Objective
Hours: Full time, Monday through Friday days with two evening shifts per week.

The Case Manager supports the mission of Churches United (to provide safe shelter, stable housing, nutritious food, and a path towards healing) by offering support to the agency function of Case Management.
Responsibilities:
- Uphold vision and goals for an effective support services and case management (CM) program.
- Provide case management and support services to shelter guests.
- Meet with each guest within 72 hours of their intake.
- Maintain knowledge of community resources and attend trainings to achieve and sustain best practices.
- Monitor daily logs and incident reports for men and women guests needs and issues.
- Prioritize urgent needs without losing track of ongoing needs.
- Identify mandated reporting issues and file appropriate reports.
- Attend daily staff huddles or as needed.
- Attend bi-weekly Case Management meetings
- Attend weekly supervisory meeting with the Director of Supportive Services
- Other duties
- Adhere to agency policies and work rules including confidentiality, child abuse reporting and code of ethics.
- Maintain professional boundaries.
- Other duties as assigned by Director of Support Services
- Maintain a supply of forms from various agencies for housing related applications

Work Environment
This job operates in a professional office setting. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.
Physical Demands
This is largely a sedentary role. Special events may require moving of tables and chairs, lifting and unpacking supplies, etc. This requires the ability to lift 20 pounds and bend or stand as necessary.
Position Type/Expected Hours of Work
This is a full-time position. Days and hours of work are Monday through Friday during standard business hours with 8 evening shifts a month. Weekend work may be required as job duties demand. Rotating on call weekends.
Required Education and Experience
- Bachelor of Social Work and LSW preferred.
- Knowledge of and sensitivity to individual struggles with homelessness, poverty, substance abuse, mental illness, and criminal records.
- A minimum of 2 years applicable experience preferably in social service setting with people in crisis.
- Ability to take initiative and to work both cooperatively with a team and independently.
- Excellent customer service, communication, organizational, and time-management skills.
- Work or life experience that demonstrates ability and willingness to work with Churches Uniteds philosophy and mission.
