Position Overview
We’re seeking a Part-Time (can turn into full time depending on performance) Home Care Service Coordinator to oversee scheduling, client and caregiver communication, and service coordination in the Denver Metro area. This role is ideal for someone who thrives in a fast-paced environment, loves helping people, and enjoys connecting resources to create solutions.
As a Service Coordinator, you’ll work closely with caregivers, clients, case managers, and support staff to ensure that client needs are met efficiently and effectively. Responsibilities
- Assess and document residents' needs, preferences, and goals to develop personalized care plans.
- Advocate for resident rights and ensure that all care practices align with ethical standards.
- Collaborate with multidisciplinary teams to facilitate effective communication among caregivers, families, and healthcare providers.
- Manage client authorizations and maintain accurate client records, including reauthorizations and new client documentation.
- Provide ongoing supervision and support to direct care staff, setting clear expectations for attendance, performance, and conduct.
- Assist with new hire onboarding, ensuring all required documentation is completed.
- Maintain accurate clinical records, tracking clients’ progress and goals.
- Support community outreach efforts and assist with marketing activities to promote the agency.
- Provide telephone and clerical support to staff as needed.

What You Bring
- Education: High school diploma or equivalent required.
- Experience:
- Experience in Home & community care.
- At least 1 year of office and/or recruiting experience.
- Case Manager experience strongly encouraged to apply.
- Skills: Strong organizational, interpersonal, and communication skills.
- Technical Proficiency: Familiarity with Microsoft Word, Excel, and Google Docs.
- Other Requirements: Reliable transportation. Bilingual candidates are encouraged to apply.
