What are the responsibilities and job description for the Social Worker position at Citizens Memorial?
Citizens Memorial Home Health Services is recruiting a Medical Social Worker, or an individual pursuing their license, to provide counseling to patients for behavioral change.
Our philosophy is to care for those in need with an integrated plan that includes a broad range of services. We minister to the physical, as well as the mental, spiritual and social needs of our patients. You will work with a set of clinics with providers and nurse care managers to coordinate and provide care.
A typical day will include: Helping the patient identify barriers and behaviors that lead to poor outcomes, and identifying goals and developing strategies or behavioral modifications to achieve goals. You are also responsible for the assessment and transition of care for patients in the Emergency Department and from Primary Care into therapy.
This position is: Part-time working 40 hours every 2 weeks. Work schedule can be flexible between 8am-5pm. Monday-Friday.
Job Requirements:
- Education: MSW or Master's in Counseling from an accredited institution
- Experience: 1 year of experience preferred
- Skills: Patient care, communication, organization, teamwork, attention to detail
- Licensure/Certification: N/A
