Job Description
Join the Street Health Team, a dynamic, multidisciplinary group dedicated to providing compassionate, community-based care to people experiencing homelessness in San Francisco. Our team includes Behavioral Health Clinicians, Nurses, Health Workers, Physicians, and Psychiatrists, community members, all working together to address the complex health and social needs of the communities we serve.
The Street Health Team works within assigned neighborhoods, meeting individuals where they are to provide essential services and support that promotes their well-being. We collaborate closely with other city agencies, including the Department of Homelessness and Supportive Housing, to connect people with shelter, treatment, and long-term support.

ESSENTIAL DUTIES
The essential functions of a Health Worker III (2587) include, but are not limited to :
- Interviews and screens patients, identifies patients' general conditions, and assists in assessing specific patient conditions and treatment planning in conjunction with professional staff; may perform crisis intervention activities
- Represents program staff in meetings with local community groups and governmental and social agencies to provide information on the activities and goals of the assigned program
- As part of a therapeutic program, may conduct craft, recreation, and other activity groups; as directed, may assist in conducting therapy sessions under professional supervision
- Maintains records incidental to other assigned duties, including patients' charts; may conduct surveys and operate technical equipment
- Supervises others, primarily in the lower Health Worker classifications
- Assists in the planning of recreational, educational, and work therapy activities within therapeutic rehabilitation programs
- When assigned to a specialized activities program, plans, implements, supervises, coordinates, publicizes, evaluates, and documents activities for patients, including social, creative, educational, physical, and religious programs
- Performs other duties as assigned
Qualifications
MINIMUM QUALIFICATIONS
EXPERIENCE : Two (2) years of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties :
Substitution : Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.
Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.
One year of full-time employment is equivalent to 2000 hours. (2000 hours of qualifying work experience is based on a 40-hour work week.)
