Position Summary :
This is a part-time 28 hrs / week professional position assisting older adults and adults living with a disability who are dealing with varied concerns. Provides person centered information and assistance regarding community resources. Completes long term care options counseling. Determines functional eligibility and completes enrollment for Familycare and IRIS, Wisconsin's Long Term Care Programs.
The position is based in the Wausau ADRC-CW Office with travel throughout Marathon County. Additional travel throughout the ADRC-CW services region for meetings and case assistance.
Qualifications :
- Bachelor's degree in Human Services, Social Work or closely related field.
- Work experience of at least one year working with older adults or adults with a disability.
- Valid driver's license and insurance; or the ability to obtain reliable transportation.
