Position Summary:
This is a part-time 28 hrs/week professional position assisting older adults and adults living with a disability who are dealing with varied concerns. Provides person centered information and assistance regarding community resources. Completes long term care options counseling. Determines functional eligibility and completes enrollment for Familycare and IRIS, Wisconsin’s Long Term Care Programs.

Provides person centered information and assistance regarding community resources. Completes long term care options counseling. Determines functional eligibility and completes enrollment for Familycare and IRIS, Wisconsin’s Long Term Care Programs.
The position is based in the Wausau ADRC-CW Office with travel throughout Marathon County. Additional travel throughout the ADRC-CW services region for meetings and case assistance.
Qualifications:
- Bachelor’s degree in Human Services, Social Work or closely related field.
- Work experience of at least one year working with older adults or adults with a disability.
- Valid driver’s license and insurance; or the ability to obtain reliable transportation.
Examples of Work Performed:
- Provide accurate information to individuals regarding available resources.
- Assist individuals to access services to meet their needs.
- Conduct Long Term Care Functional Screen to determine eligibility for Familycare and IRIS.
- Provide assistance to individuals with Long Term Care Medicaid applications.
- Complete enrollment into the Long Term Care programs of Familycare and IRIS.
- Provide disenrollment counseling to members of Wisconsin Long Term Care Programs.
- Complete required documentation into client tracking system.
- Refer customer to other agencies and services when appropriate.
- Complete Outreach to target populations and community agencies.
- Develops and expands job related skills and knowledge by attending training.
- Completes Meals on Wheels program assessments.
- Participates in agency committees and community groups.
- Complete and submit all required paperwork timely and accurately.
- Maintains regular and predictable attendance.
Knowledge, Skills & Abilities:
- Ability to obtain Functional Screener and Options Counseling certification within 3 months of date of hire.
- Ability to work effectively with older adults and adults with disabilities.
- Ability to conduct assessments of customer needs by telephone or in person interviews.
- Skills in interviewing individuals in a one on one or group setting.
- Excellent interpersonal, verbal and written communications skills.
- Ability to develop and maintain rapport with community partners.
- Ability to communicate detailed and accurate information to individuals and groups.
- Knowledge of community resources that assist older and disabled adults.
- Ability to research, analyze and resolve problems.
- Ability to listen and respond in a nonjudgmental and respectful manner.
- Ability to work independently and exercise judgment in accordance with established procedures.
- Ability to travel to individuals homes and community locations.
- Ability to be organized, multitask and meet deadlines.

- Adhere to confidentiality requirements.
- Knowledge of and commitment to adopting a trauma informed approach to practice.
- Skills in utilizing a computer using a variety of software, email and internet.
