Job Description

 

Job Title:  Case Manager                   Reports to:  Director of Operations             

 

PURPOSE:  To plan, organize and implement social work intervention for homeless clients and their families in order to resolve special problems facing them and to improve their ability to transition back into society.

 

ESSENTIAL FUNCTIONS-JOB DUTIES:  The following duties are considered essential to the job.  Qualified individuals must have the ability to perform the following duties.


  1. Perform an intake evaluation including a social history on all City Mission clients.

 

  1. Establish an individualized housing and supportive service plan for each client (Client Treatment/Service Plan).

 

  1. Assist clients and families in locating and utilizing community resources.

 

  1. Maintain relationships with a wide variety of community agencies and facilitate communication between them and clients, their families and clinical personnel. This would include assisting clients and their families in accessing benefits such as Medicare, Medical Assistance, Veteran’s Benefits, OVR benefits, Public Assistance, and others.

 

  1. Establish and maintain case files for all clients according to guidelines established by City Mission.

 

  1. Evaluate social, emotional, financial, and environmental needs of clients. Counsel, advise, and support clients and their families.

 

  1. Evaluate progress of every individual to determine progress towards initial goals set. Based upon this evaluation, provide input to the Director of Operations to determine housing extenstions.

 

  1. For residents of the Gallatin School Living Centre, provide individual weekly case management services, monthly case management team meetings with clients, assist other case management staff in coordinating workshops, life-skills programs, home-ownership, savings programs and other activities for clients and their families, as directed by the Director.

 

  1. Coordinate statistical research, compile reports, attend scheduled case management team meetings, and facilitate staff training as directed by the Director of Operations.

 

  1. Perform an exit interview with clients as well as a client satisfaction survey on all clients leaving City Mission programs. For clients who have left the emergency shelters, provide a follow-up at 6 months. For clients who have left the Gallatin School Living Centre, and gone on to permanent housing, provide follow-up case management services for a period of 6 months.

 

 

            SECONDARY FUNCTIONS-JOB DUTIES: The following duties are considered secondary to the essential functions listed above:


  1. Document social histories as needed.

 

  1. Intervene in emergency situations

 

  1. Present case material and contribute to discussions in staff meetings.

 

  1. Participate in community-related functions as a Social Service representative of City Mission.

 

  1. Assist in resolving social disruptive, client-oriented situations when requested, including child abuse, adoption, custody issues, law-enforcement cases, Emergency Medicine and Family Health Service cases.

 

  1. Actively participate in annual fundraising events for the agency.

 

 

            JOB REQUIREMENTS:


  1. EDUCATION/KNOWLEDGE REQUIREMENTS:


                        Specific Education, Certification, and/or Licensure:

           

                        Bachelor of Social Work (BSW) or related field.  Preferred MSW or

                        related field.  Position requires much flexibility and the ability to

                        occasionally work beyond 9-5.

 

                        Act 33, 34 clearances

             

  1. Essential Job-Related Experience:


One (1) year of social work experience

 

 

  1. Preferred Job-Related Experience:


One (1) year of case management work with homeless population

 

            SUPERVISORY RESPONSIBILITY:  Position may require minimal

            supervision of student interns and volunteers.

 

            WORKING CONDITIONS:  Position involves possible exposure to the

            following harmful elements.
