Clinician II will include collaboration with the Chesapeake Correctional Center and, local providers, community agencies, and program participants to provide ongoing support to eliminate fragmentation, duplication, or gaps in care and treatment.
The Clinician II will be discharging Chesapeake Residents from the State Mental Hospitals, to include setting up aftercare and developing Discharge Assistance Program (DAP) Plans, if appropriate. This position will work with primarily Civil and Safety Net Patients
Other related tasks for the clinician will include:
The Clinician II will work the Discharge Assistance Project (DAP) budget and the Office of Census Reduction to facilitate consumer discharges from the state mental health facility.
The Clinician II will facilitate the discharge of consumers from the State Mental Hospitals.
Establishes and maintains inter-agency relationships and other supportive community services.
Maintains records according to existing standards and policies and to ensure protection of client confidentiality and conducts quality reviews of all charts.
The Clinician II serves as an advocate on behalf of the consumers by making appropriate referrals to other social services, education, and health agencies as needed.
Provides service techniques including rapport-building strategies, facilitating meeting basic needs, and motivational interviewing techniques.
Provide overall service coordination, referral, linkage, and advocacy to consumers with varying needs within the Forensic department.
Performs other related duties as assigned.

Required Qualifications
VOCATIONAL/EDUCATIONALREQUIREMENT:
Requires a bachelor’s degree in social work, psychology, closely related field, or another degree as approved by the Virginia Department of Health Professions.
EXPERIENCEREQUIREMENT:
In addition to satisfying the vocational/education standards, this position requires a minimum of two years of related, full-time equivalent experience.
SPECIALCERTIFICATIONS:
Requires a valid driver’s license and a driving record that is in compliance with theCity Driving Standards.

Assignment requires Qualified Mental Health Professional (QMHP – Adult )
CPR, First Aid within 3 months of hire
SPECIALREQUIREMENT:
Employees may be expected to work hours in excess of their normally scheduled hours in response to short-term department needs and/or City-wide emergencies. Emergency operations support work and work locations may be outside of normal job duties.
Preferred Qualifications
Four Years experience in mental health and substance abuse counseling, to include case management experience.
State hospital Discharge Planning experience preferred
