The Central Ohio Area Agency on Aging (COAAA) plans, funds, and delivers social services that help older adults and individuals with disabilities remain safe and independent in their homes. With the assistance of area providers, COAAA arranges and coordinates services to help individuals with daily living such as homemaking, transportation, home-delivered meals, and personal care. COAAA offers education and resources to caregivers, professionals, and the public and advocates for programs and policies that benefit older adults and individuals with disabilities. COAAA manages services for 10,000 people and funds services for another 20,000 older adults in eight central Ohio counties.

As a Case Manager you would be expected to:
· Fulfill the needs and the requirements of your respected program
· Assess an individual’s needs for care
· Create appropriate care plans
· Complete home visits
· Collaborate with clients and other team members
· Coordinate services of care with outside entities/agencies
· Learn various computer systems
· Adapt to a changing work environment
Required Minimum Qualifications:
· Possession of a valid Motor Vehicle operator’s license
· An active LSW for the State of Ohio
Preferred Qualifications:
· One year of case management experience
· One year of experience completing home-visits
· One year of connecting individuals to resources and services
· Experience working with Microsoft Excel
· A Bachelor’s degree or higher
