MOS Code: 0169 (Navy), 47G1D (Airforce), 66B (Army)
Education and Experience: High School Diploma, GED or higher plus two (2) years' experience in a public health setting or as a community service aide, employment aide, social services aide, manpower aide, community worker, community organizer or outreach person.
Licenses and Certificates: Texas Community Health Worker Certification within six (6) months of appointment. Valid Texas Class "C" Driver's License or equivalent from another state.
General Purpose
Under general supervision, provides community health services to the target population through education on preventative health issues, laboratory testing interpretation, contraception counseling, monitoring client services and referrals, testing and access to care.
Typical Duties
Conduct client-centered, community health services in coordination with caseworker. Involves: Identify target population gatekeepers, outreach sites, and conduct community outreach as requested. Educate patients and families about communicable/infectious diseases such as HIV/ STDs, Zika, mosquito borne diseases, and applicable prevention methods. Provide contraception counseling to women of childbearing age and their partners to include preventative health education on Long-Acting Reversible Contraception (LARCs). Interpret laboratory testing and facilitate contact notification for cases. Conduct and monitor medical referrals in coordination with caseworker. Assist in the development of educational materials for public dissemination. Maintain records and documentation on all activities and compile reports as needed.
