About BHRT
The Behavioral Health Response Team (BHRT) has been specially trained to handle incidents involving persons with mental illness and those in crisis with care and expertise, ensuring such persons receive appropriate responses based on their needs.

Primary Duties / Brief Description of Work :
The Community Outreach Coordinator is responsible for :
Outreach Coordinator will follow up on referrals made from GPD officers or other sources and will make proactive contacts with individuals identified as frequent 911 callers. Follow-up may be over the phone or in person and could include, but is not limited to, providing education about the purpose of the BHRT, providing information about local mental health / substance use resources, linking people to resources for basic needs, coordinating with providers and supports, etc. This individual will be expected to travel to individual's homes, homeless shelters, and other locations in the community in order to make contact. The outreach coordinator will be expected to operate independently and in consultation with the Team Lead in order to provide appropriate follow-up and referrals. Outreach Coordinator will coordinate with other community resources for care coordination and may participate in resource fairs or other community events on behalf of the team.
Qualifications
Minimum Qualifications :
- Associate degree with 4-6 years of experience in case management, mental health, substance use, or related field
