City of Hope Orange County is transforming cancer treatment by bringing world-class expertise and specialized care closer to home. Supported by a regional network of care, our 11-acre academic cancer campus in Irvine is home to City of Hope Orange County Lennar Foundation Cancer Center, our outpatient cancer center, and Orange County’s only cancer specialty hospital, opening in late 2025.
With the opening of the hospital, the full continuum of advanced cancer care—from prevention through survivorship—will be available in Orange County for the first time, eliminating the need to travel for lifesaving treatment. Seamlessly connected to our outpatient cancer center, the new clinical research hospital will feature 73 inpatient beds, advanced surgical suites, groundbreaking clinical trials, and holistic services such as rehabilitative therapy and spiritual care. This isn’t standard cancer care. This is lifesaving cancer care. This is Hope.

The Program Manager, Medication Safety & Quality is a licensed pharmacist who will work closely with the Enterprise Director, Director of Medication Safety and report directly to the local Director/Executive Director. The Program Manager will be responsible for leading   medication safety related strategies, goals, and policies/practices, and to ensure that organizational standards are disseminated and understood by all staff members - promoting a culture where medication safety is a priority.  
The Program Manager works collaboratively with multiple disciplines (e.g. medicine, nursing, pharmacy, respiratory therapy, radiology) to proactively incorporate best practices from expert organizations (e.g. The Joint Commission, The Institute for Safe Medication Practices, Agency of Healthcare Research and Quality, National Quality Forum, The Leapfrog Group, United States Pharmacopoeia, California Board of Pharmacy, California Title 22, Medicare Conditions of Participation…) to reduce the potential for medication errors and medication near misses.  

In addition, the Program Manager is responsible for:
•    conducting retrospective reviews of occurrence reports, pharmacist intervention reports, and other data of actual and potential medication errors for trends and opportunities for process improvements;
•    leading medication safety prospective assessments and identifying opportunities to improve medication safety and developing and/or revising processes, policies and procedures focusing on best practices;
•    leading pharmacy root cause analysis and/or failure mode effects analysis, participating in enterprise-wide similar activities whenever the analysis involves safe medication practices;
•    educating other health care professionals regarding contributory system failures that result in medication events and leading process improvements to minimize the potential for future medication events
•    act as coordinator for inspections and surveys


As a successful candidate, you will: 

  • Identifies best practices for medication safety, and incorporates best practices into processes, policies, and procedures.
  • Analyzes local processes that may contribute to medication errors and near misses and implements proactive measures, processes, polices, training to prevent them. 
  • Analyzes occurrence reports involving potential or actual medication errors for trends and opportunities for improvement and lead efforts to improve the systems.
  • Works with the Enterprise Director, Director of Medication Safety and multiple disciplines to incorporate best practices and process changes to eliminate the potential for medication errors.
  • Effectively communicates and educates all internal and external customers on all medication safety issues.
  • Maintain medication safety metrics, including the medication safety dashboard.
  • Promotes collaboration and teamwork among all members of the healthcare team. 
  • Maintains current knowledge, skills and competencies through involvement in continuing professional education activities.
  • Monitoring of medication safety alerts from external bodies (ISMP, ASHP, etc.) and coordination of internal review and follow up.
  • Precepting pharmacy students and residents.
  • Support of a Just medication safety culture with pharmacy enterprise and with all clinical staff.

Your qualifications should include: 

  • Graduate of an ACPE-accredited School of Pharmacy with a Pharm D
  • Minimum 1 years of experience conducting medication safety duties
  • Current CA Pharmacist License    
  • Preferred Experience--PGY2 residency in medication safety ASHP/ISMP medication safety certification, Previous experience in an academic hospital setting, Oncology health care system experience
