- Supervises, evaluates, and develops assigned staff, including hiring, conducting performance reviews, scheduling, approving time-off, providing training, addressing performance issues, and making recommendations regarding staffing and compensation.
- Oversees the administration of chronic disease prevention programs, with a focus on lifestyle interventions and community health education. Educates patients on various health programs
- Coordinates educational outreach and ensures that best practices and health regulations are followed. Leads efforts to increase awareness of chronic disease prevention through community engagement, includes planning workshops, developing informational materials, and facilitating patient enrollment in prevention programs.
- Prepares grant applications, manages funds, and ensures compliance with grant conditions. Responsible for regular reporting on program outcomes and financial management to funding agencies.
- Monitors and evaluates program effectiveness, making adjustments to improve service delivery. Prepares regular reports for stakeholders based on program metrics and outcomes.
- Is responsible for developing a workflow for the capture and timely submission of all program data into relevant systems
- Is responsible for developing a workflow strategy based on data to accomplish program objectives
- Will be required to drive City vehicle for City business use
- Performs other duties as assigned

- Bachelor's degree from an accredited* college or university in Public Health, Nutrition, Health Education, Community Health, or a related field
- At least one (1) year of experience in case management, program development, data analysis, interdisciplinary collaboration, or health education

- Valid Texas Driver License. If applicant holds an out-of-state license, a State of Texas Driver License must be obtained within 6 months of employment.
As part of the minimum requirements for all positions with the City of Laredo, a thorough background check and certified school transcripts will be required from all applicants who are offered employment.
