The City of Virginia Beach Department of Human Services is seeking to hire a motivated Crisis Management State and Regional Hospital Discharge Planning Clinician who has experience providing discharge planning services and community crisis stabilization services to adults with serious mental illness, substance abuse use disorders or co-occurring disorders for the Crisis Management Unit in the Behavior Health Division.

Responsibilities:
- Provide local and state hospital discharge planning services as well as community crisis services to adult non-hospitalized individuals experiencing an acute psychiatric crisis to avert hospitalization or re-hospitalization, by mobilizing resources of the community and natural support systems for ongoing maintenance and rehabilitation.
- Complete complex assessments, treatment planning, crisis planning, clinical intervention, consultation, supportive counseling, transition planning and other duties in a Behavioral Health setting.
- Contact the hospital, CSU, or CRC to begin discharge planning services the day the referral is received.
- Meet with assigned Community Stabilization clients the day the referral is received from VBPD/CIT/ MCRT as part of the Mobile Community Stabilization Team under Marcus Alert.
- Provide authorization of LIPOS and DAP funding when facilitating local and state hospital discharge planning.
- Complete clinical documentation and assures that all filing of clinical information and submission of billing information are done accurately and within the time frame required.
- Complete and final approve progress notes in the Electronic Health Record (EHR) within 24 hours of service being provided.
- Adhere to Agency/ DMAS/ Licensure documentation standards as documented by QA reviews.
- May serve specialized populations and/or serve in specialized roles.
- Assist with Civil Commitment Court proceedings as needed.

**Hiring bonuses shall only be for new hires of original employment or reemployment as defined in Human Resources Policy No. 5.0.
- $1,000 will be awarded in the first paycheck.
- $1,250 will be awarded upon completing the six (6) month interim performance evaluation with an overall rating of “Meets Expectations”.
- $1,750 will be awarded upon completing the employee’s one (1) year probationary period and a performance evaluation with an overall rating of “Meets Expectations”.
