Education / Experience :
1. Must possess a minimum , high school diploma or equivalent
2. Graduate of an accredited course in dietetic training , approved by the Academy of Nutrition and Dietetics
3. Must have a minimum of 1 yr experience in a supervisory capacity in a hospital, skilled nursing facility or other related medical facility
4. Must have a Sanitation Certificate approved in the state of IL
5. Must be able to read, write , speak and understand the English language
Knowledge / Experience :
1. Knowledge in developing preliminary and comprehensive assessments of the dietary need of each resident thru interviews and diet / medical history and periodic visits of residents to be able to evaluate the quality of meals served, obtain their likes and dislikes.
2. Knowledge in developing a written dietary plan of care, review and revise care plans and assessments as necessary, quarterly and annually
3. Coordinate with the Dietary Manager / Food Service Director in processing diet changes and new diets as received from nursing services
4. Attends care plan meeting and able to lead the NAR ( nutrition at risk) meetings
5. Assists in developing diet plans for individual residents
6. Reviews diet plans and menus to assure that they are in compliance with the physician orders
7. Knowledge on written dietary policies and procedures and able to interpret them .
8. Assists the in developing, implementing appropriate plans of action to correct dietary deficiencies
9. Reports to the dietary Director with ovewrsight from the Registered Dietitian residents at high nutrition risk like GT feeding, renal, Pressure injuries , significant weight changes ,etc. with regards to their plan of care and recommendations
10. Other duties as assigned by the Administrator and / or Dietitian
