Job Description
- Maintain a caseload of 10 children and families as assigned by the Program Coordinator
- Meet with each child according to contract standards (must meet at least twice a month, including one visit within the resource parents' home if the child is in placement or according to aftercare standards, if applicable)
- Meet with resource parents at least once a month, ensuring compliance with state and contract regulations as well as providing support in meeting case planning goals
- Meet with birth parents at least once a month (based on their schedule) and provide assistance, referrals and support in meeting case planning goals
- Maintain accurate, detailed and current case records on each client, prepare quarterly reports and document case sessions
