Resident Care Coordinator – Job Description
The Montclair offers a continued, higher level of support for our elderly who are no longer able to live independently. Our focus and goal are person-centered care with a high level of engagement and a focus and individualized attention to our residents in compliance with their care plans. The Resident Coordinator supports the Clinical Director and Administrator, encourages CMAs to perform tasks safely, prioritizing autonomy, engagement and independence. The right person for this role will have the ability to focus individual care without sacrificing their vision for task completion for the day, the ability to work with a team to accomplish goals, the ability to communicate with care, consideration and professionalism with both elders, teammates and family members, and the ability to serve as a true partner in caregiving for our residents. Reports: Directly to Clinical Director
In addition to tasks described in the Medication Aide job description the Lead Med Aide will also be responsible for the following activities:
- Communicating concisely and effectively with patients, families, staff, pharmacists, and other health care providers related to patients.
- Must be detailed oriented
- Have a positive attitude, be highly approachable, and demonstrate excellent listening skills. Good judgment, initiative, and sound analytical skills are other vital qualities that a successful team manager should have.
- Comply with the job description for CMAs
- Team managers should lead by example, be highly organized, and have excellent time management skills. They must also show excellent judgment, being decisive and willing to take risks when the situation warrants.
- Report resident, staff and vendors concerns timely to Clinical Director or Administrator for all concerns to ensure timely resolution.
- Comply with HIPPA regulations
- Scheduling to ensure the community staffing needs are met and maintain consistent agency staff as reasonably possible
- May have to fill in for openings related to open shifts, call outs as needed
- A team manager should possess a unique set of skills, have relevant education and training, and exhibit essential personal qualities to be successful. By having these attributes, a team manager can successfully navigate the complexities of the role and lead their team towards achieving results.
- Must be able to handle potentially stressful situations and multiple tasks simultaneously including instructing and staff and residents
- Acts as a mentor for new staff
- Keeps Clinical Director informed of changes in cares being given to residents
- Does periodic observations of cares provided only as assigned by the Clinical Director to assure that policies/service plans are being followed
- EMR audits for compliance
- Train new CMAs
- Conduct cart audits with CMA
- Administering medication to patients in accordance with physician orders
- Monitoring and recording patients’ symptoms and vital signs
- Reporting and documenting any changes in patients’ conditions to medical professionals
- Ensuring proper handling and storage of medication
- Refilling and organizing medication carts, as needed
- Maintaining inventory and ordering medication supplies
- Quality check of medication room monthly
- Maintain a clean and organized nurses’ station and office
- Ensure copies are readily available for all forms
- Participate in new hire orientation
- Acts as LEAD supervisor in the absence of Clinical Director or other supervisory personnel.
- Assists and directs CMAs to ensure efficient, prompt and quality care is provided to residents
- Monitor and assist agency staff to ensure compliance with care and services
- Assist Clinical Director with specialized task training
- Assist with wellness day, vendor visits etc.
- Administering medication to patients in accordance with their care plan
- Monitoring and recording patients’ symptoms and vital signs
- Auditing reporting and documentation of any changes in patients’ conditions to medical professionals and responsible parties
- Ensuring proper handling and storage of medication
- Refilling and organizing medication carts, as needed
- Maintaining inventory and ordering medication supplies
- Provides direct resident care, following the residents care plan and physician orders
- Display ingenuity, self-reliance, resourcefulness, assertiveness, ambition
- Ensure accurate accounting and safekeeping of all narcotics at all times
- Motivate team lead with enthusiasm and setting a good example for others
- May be asked to participate in the interviewing process for new hires
