Description
The Referral Specialist oversees the overall Placement and Retention of all Members in the Residential facilities. Responsibilities include the following functional areas include market open beds, assessments for appropriateness, member retention plans, etc.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Utilize marketing strategies to secure prospective referrals and residents.
- Welcome and show residential facilities to prospective new residents, funders, care managers, guardians and other community stakeholders.
- Lead weekly Openings meeting.
- Assess potential admissions in person or virtually (when needed).
- Respond timely to all verbal or electronic inquiries from referral sources, prospective residents, guardians.
- Ensure all new resident and funder paperwork is completed.
- Ensure residential facilities are occupied to their fullest capacity.
- Continually monitor and analyze referral sources, conversion ratios, budget guidelines, retention information and marketing data to be able to give up-to-date- and proper information when requested by the Leadership Team.
- Design, develop and implement resident retention strategies and programs, globally or individually.
- Team up with Accounting on Revenue Rates, Ideal Homes & Staffing Patterns in order to achieve financial goals.
- Maintain awareness of market and industry conditions and trends in residential vacancies via trade publications, professional organizations and associations, etc.
- Upon residential admission, ensure all employees are updated and informed on home, employee, agency, procedural and consumer support process issues and changes in order to provide proper care to consumers.
- Assist with new Member move ins.
- Work with Residential Management to develop/revise consumer service plans, staffing patterns, follow budgets, and monitor expenses to ensure Clarity Care and consumer needs are met.
- Schedule first Individual Support Plan meeting with Clarity Care and outside Care Team.
- Complete initial Individual Support Plan for new Member. Ensure all ISP’s are in compliance and initial 30 day and 6 months ISP meetings are taking place.
- Train new Residential leaders on Individual Support Plan creation as needed.
- Monitor establishment and maintenance of good relationships with members of the Care Team, employees, guardians/family members, funding agencies, other service providers and the community at large to uphold Clarity Care’s values of care and compassion and commitment to quality and excellence.
