Key
Responsibilities: 1.
Skilled Care Admissions & Care Planning, including coordination with care managers at hospitals
and other referral sites, collaboration with residents, families, and the
healthcare team to facilitate smooth admissions to skilled care, conducting initial assessments, reviewing and updating care plans regularly, and providing education and guidance to residents
and families on care processes, community resources, and transitions of care. 2.
Memory Care Family Support, including serving as a primary point of contact for
families with loved ones in memory care, offering emotional support, guidance,
and resources to address their unique needs, facilitating regular family meetings to discuss
care plans, and address concerns, leading family support groups and educational
sessions, partnering with the memory care team to design
and implement care strategies that enhance residents’ quality of life and
family satisfaction, and to provide education and support regarding the Montessori approach to memory care. 3.
Wellness Coaching for Mental Health, including offering individualized wellness coaching
services to support residents’ mental and emotional health, developing and facilitating wellness programs, support groups and
workshops focused on mental health topics, collaboration with the community’s healthcare
and wellness teams to ensure a cohesive, interdisciplinary approach to
residents’ mental well-being, and conducting mental health assessments, providing short-term counseling as needed. 4. Resident
Services Team Member, including assisting with new resident move-ins, like assessments, compiling forms, etc., and assisting residents and families with
