About your role :
Are you a Qualified Professional who wants to a part of the NC Medicaid transformation and serve our clients in need? Our Care Managers are Qualified Professionals who deliver Tailored Care Management services to clients with complex behavioral health and healthcare needs such as mental illness, substance use disorders, intellectual and developmental disabilities, and / or traumatic brain injuries. Support for these clients includes housing, employment, food insecurity, physical health, finances and safety which may be affecting their well-being. In this role, you will lead a team of Care Manager Extenders through service provision to meet or exceed the established key performance indicators and desired client outcomes.
This role includes travel up to 50% or more of the time, to serve our clients where they are in their journey.
Does the following apply to you?
- Bachelor’s Degree in a related field (health, psychology, sociology, social work, nursing, or another relevant human services area) AND 2 years of experience working with the MH / SU / IDD population
- Bachelor’s Degree in an un-related field (health, psychology, sociology, social work, nursing, or another relevant human services area) AND 4 years of experience working with the MH / SU / IDD population
- Master’s Degree in a related field (health, psychology, sociology, social work, nursing, or another relevant human services area) AND 1 year of experience working with the MH / SU / IDD population
- Full or provisional licensure or certification with the appropriate North Carolina board (LCSW, LCMHC, LCAS, LPA, LMFT, etc.)
- Registered Nurse (RN) in North Carolina AND 4 years of experience working with the MH / SU / IDD population
