SUMMARY

The Health Navigator coordinates with hospital systems and others in the community to form professional relationships and reduce barriers for potential clients. The Health Navigator coordinates with these outside agencies/facilities, in order to engage difficult to reach individuals and potential new members. The Health Navigator contacts potential THL (Tennessee Health Link) clients and educates them on how the service could be beneficial in addressing his or her unique needs.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Establishing and maintaining successful partnerships and relationships with hospital systems,

emergency departments, physician`s groups, inpatient facilities, and others in the medical and behavioral health community and arranging and attending meetings with these groups as needed.

• Working from a list of potential clients, making outbound calls to educate potential clients on

Clarvida's THL program, and persuading them to enroll in the program.

• Displaying creativity and persistence in attempting various other methods of contact when outbound calls prove to be unsuccessful for certain individuals. This may include, but is not limited to, sending out correspondence by mail, attempting face to face contact at a potential client's home, visiting a potential client at a hospital or emergency department, contacting the MCO or other treating professional to obtain current contact information, etc.
• Serving as a liaison and client advocate with other professionals and agencies involved in assigned clients’ care.
• Attending weekly supervision and team meetings and making presentations on accumulated data and statistics as required.
• Supporting fellow team members to ensure the best outcomes for all Clarvida clients and families. Completing clinical documentation and data entry within established time frames and rectifying all deficits within the allotted time frame.
• Contributing to the Performance Quality Improvement Cycle, including the collection and use of data to continuously improve client and program services.

TRAVEL

This position requires travel of approximately 50% of the time.

REQUIRED EDUCATION AND/OR EXPERIENCE

• Bachelor`s Degree (Social Work, Psychology, Mental Health, Child & Family Studies or Human Development) or LPN (Licensed Practical Nurse)

PREFERRED EDUCATION AND EXPERIENCE

• Experience working in the healthcare industry. Background in marketing, sales, or public relations is a plus.

CERTIFICATES, LICENSES, REGISTRATIONS

Valid driver’s license and automobile insurance in good standing

QUALIFICATIONS

• Language Skills

Ability to read, analyze, and interpret the most complex documents. Ability to respond effectively to the most sensitive inquiries or complaints. Ability to write speeches and articles using original or innovative techniques or style. Ability to make effective and persuasive speeches and presentations on controversial or complex topics to top management, public groups, and/or boards of directors.

  Mathematical Skills

Ability to apply advanced mathematical concepts such as exponents, logarithms, quadratic equations, and permutations. Ability to apply mathematical operations to such tasks as frequency distribution, determination of test reliability and validity, analysis of variance, correlation techniques, sampling theory, and factor analysis.

  Reasoning Ability

Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to

interpret an extensive variety of technical instructions in mathematical or diagram form and deal with several abstract and concrete variables.
