Position: ACT Case Manager
Program: Open Door
Reports to: Intensive Outpatient Services Program Manager
Status: Non-exempt
Pay Scale: Based on current Union salary scale
POSITION SUMMARY
This position will be part of the ACT team providing community outreach services to individuals who struggle in traditional mental health services to find recovery. Individuals served often end up hospitalized, homeless and/or jailed as a result. Consequently, the ideal candidate will be hard working, energetic, tenacious, optimistic, have a good sense of humor and caring in their approach to services. They will be able to work as part of a small team of professionals and be comfortable serving individuals in their homes and community settings.
Essential Functions / Major Responsibility
- Provide case management services to assigned clients to include community outreach and skills training as needed.
- Establish appropriate and supportive relationships with ACT clients
- Provide individual and family education about mental illness as needed.
- Maintain all required paperwork / charting requirements as established by agency contracts
Secondary Functions
- Assisting in finding and obtaining resources in emergency situations.
- Helping with resources for housing and shelter, assisting with finding resources for security deposits, etc., enhancing ability to maintain housing, helping to locate and facilitate supported/assisted living or community resources for in home care.
- Teaching housekeeping, cooking and shopping skills, helping to arrange for housekeeping supports.
- Assisting with locating and learning how to use available transportation.
- Teaching and helping to improve living, coping, symptom management, communication and interpersonal skills.
- Collaborating and coordinating care with other treatment providers and helping connect with legal, court, and community resources.
- Providing assistance with crises and helping with development of crisis plans before the need arises.
- Helping clients connect to community health care providers and assisting with medical and dental appointments.
- Offering education about mental illness and treatment to clients, family members and other significant members of the client's support system.
- Providing support and encouragement in client's ability to work, go to school, develop friendships and extend the social network.
- Advocating for client's rights and fair treatment in all areas of life.
- Maintaining all required paperwork/charting requirements as established by agency contracts.
- Achieving and maintaining agency standard productivity requirements.
Education and Experience
Bachelor's degree specializing in Mental Health, Social Work, Psychology, or Human Services. A minimum of three years of experience in the social service field with mental illness or chemical dependency is preferred.
Minimum Requirements
Successfully pass criminal history check. Provide proof of current, valid Driver License, comprehensive automobile insurance, and a safe driving record to use agency or personal vehicle for required duties. Use active listening skills with clients, colleagues, and community partners. Consistently present a positive, cheerful attitude, and professional dress and demeanor that reflect favorably upon the organization’s image. Handle frequent interruptions and changing priorities. Use effective customer relations skills, including the ability to work with diverse people in a stressful environment, displaying an understanding of group dynamics and dealing with stress effectively.
Practice strong organizational skills and effective use of time.
Physical Demands
While performing the duties of this job, the employee is frequently required to sit, talk, and hear. The employee is regularly required to travel between sites in Clatsop County and may be required to drive. The employee is regularly required to move about the facility and may be required to use stairs; use hands and fingers to operate, handle, or feel objects, tools, or controls; and reach with hands and arms. Requires sufficient mobility and balance to participate in ProACT (Professional Assault Crisis Training) training and to implement its principles.
Hazardous Conditions
Risk of verbal and physical threats from clients. Use of keyboard and computer monitor (repetitive hand motions and potential eye strain). Possible exposure to airborne pathogens.
Working Conditions
Primarily in clients’ natural environment; generally spends at least 80% of time in the field.
Equipment/Materials Used
Common office machines including basic computer programs, printer, telephone equipment, copier, and facsimile. Drives agency or personal vehicle.
