This position qualifies for a hire on bonus of $1000 with 1/2 paid with 1st check after hire and 1/2 paid with 1st check following 6-month probationary period.
Clatsop Behavioral Healthcare is moving this position to a 4 day 9 hour work week with paid lunches to promote a better work life balance.
Clatsop Behavioral Healthcare has an immediate opening for a full time Medication Assisted Treatment (MAT) Care Coordinator. This position will perform a variety of services in an outpatient setting to provide direct services to the adult and child population. The Care Coordinator will possess administrative knowledge, including proficiency in use and management of an electronic medical record; and customer service knowledge, including compassionate and professional interpersonal interaction with both clients and coworkers. This position will be able to function as part of an inter-disciplinary team and possess flexibility to assist fellow team members to provide continuity and consistency of care to the agency's clients. Additionally, this position will provide behavioral health screening via telephone in order to schedule new clients and screen referrals for services. For clients who are eligible for Oregon Health Plan and need to enroll, the position will serve as a certified OHP assister.
Essential Duties and Responsibilities:
· Assist any Clatsop County resident with enrolling in OHP (after completion of assistor training)
· Screen and triage new client requests for services, conduct intakes, schedule assessments, and ensure continuity of referral/intake process.
· Screen and triage referrals from outside agencies.
· Act as liaison with hospitals and other outside specialty care settings.
· Provide information and referrals to community resources for clients requesting services as needed.
· Document screening information in the medical record and gather additional information as needed
· The ability to remain flexible to the changing demands of a mental health system and the needs of the agency.
Additional Duties:
· Other duties may be agreed upon or assigned based on the needs of the agency to provide the best possible care to clients, families, and the community.
· Document all client interactions, appointments, and sessions.
· Assist lead with building relationships with community partners.
· Assist with internal and external referrals, including the peer support specialist.
· Updating of medication changes and additions, in the EMR and with Millennium urinalysis tech.
· Assist clients with medication refills and appointments with the MAT prescribers (med-management).
· Assist clients with establishing a PCP, as required by the MAT requirement for care.
· Provide case-management and references for items such as Employment, GED, NOHA and education, such as Lives in Transitions.
Requirements
- Ability to be become certified OHP assister.
- Meet one of the following credential requirements
- CADC – Certified Alcohol and Drug Counselor in Oregon
Preferred
- Prior substance use disorder treatment background and/or lived experience with or in the recovery community.
