POSITION PURPOSE: The Health Services Director (Registered Nurse: RN) is employed full time and is directly responsible to the Administrator for ethical, professional, and adequate delivery of health services to residents. He/she will also facilitate and coordinate all health services employee educational requirements in accordance with State and Federal licensure for Assisted Living.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following:

Makes every effort in good faith to support the success of every employee in the Clatsop Care Center Health District (including but not limited to peers, subordinates, supervisors and district administration) Makes every effort in good faith to support the holistic well-being of residents at Clatsop Retirement Village. Functions within the scope of practice for a Registered Nurse in the State of Oregon. The RN shall participate in the development and implementation of any facility policies that affect the delivery of health care services. Delegates to appropriate staff nursing tasks in accordance with the rules of the Oregon State Board of Nursing and the Oregon Administrative Rules for Assisted Living. Ensures that all medications and treatments are given in accordance with Physician Orders and coordinates the medication delivery system. Including assuring that quarterly recaps are accomplished along with RSC. Meets with Consultant Pharmacist and facilitates actions in accordance with recommendations along with RSC. In the event the consultant pharmacists is unavailable, reviews all medications and treatment at least every 90 days.

Collaborates with RSC in review and implementation of pharmacy recommendations. Consults on residents with behavioral symptoms before psychoactive medications or restraints are requested by the facility. Implements PRN parameter sheets, and Pink Sheets on any psychotropic medication. Coordinates and facilitates a quality assurance program for resident health care services and collaborates with RSC. Participates with the facility administrator and other department directors in development and maintenance of practices and procedures that promote infection control, fire safety, quality assurance and hazard reduction.

Facilitates educational programs in accordance with licensure and facility needs, including new employee orientation and annual in-service requirements, and monthly resident education. Is available 24/7 by telephone for staff consultation – returns phone calls in timely manner. Participates with the Administrator in the hiring, coaching, disciplining and termination of Team Leaders. Directs, supervises, and evaluates (Medication Aides/Team Leaders). Reviews resident’s individual service plan along with RSC in accordance with rules for licensure and assures services are delivered in accordance with mutually agreed upon plan, when resident is receiving nursing services or experiences a significant change in condition.

The RN is knowledgeable of the general health and well-being of resident regarding in the facility. Participates with RSC in coordinating on-site health services and reviews service plan changes made as a result of provision of on-site services. Approves facility system for tracking controlled substances. Assists Medication aides in the destructions of medications. Participates in negotiated managed Risk Agreements when residents choose activities that are contraindicated to good health and safety practices along with RSC and Administrator. Reviews resident accident and incident reports and assures that established procedures are followed after completed by RSC. Will Assist RSC when there is an increase in volume. Conducts an Admission Assessment to assure residents are placed at appropriate levels and that the facility has the ability to meet their need requirements, along with HSC and Administrator. RN will ensure that all necessary documents are included in the chart prior to a resident moving in. Participates with the facility administrator in making budget recommendations and facilitates budgetary compliance when necessary. The RN shall be informed regarding resident’s conditions, including when a significant change in a resident’s condition warrants nursing or medical intervention. Assures that health services staff are informed of resident change in conditions. Participates in those committee meetings as assigned by the Administrator.

Provides Hepatitis B shots to staff and monitors for TB and other health conditions as necessary. Medication Error analysis and reporting. He/She will also teach and counsel staff regarding these errors if it is required. RN will coordinate with other entities when a resident is out of facility for health reasons. Coordinates and schedules physician’s appointments for residents with acute conditions or changes in conditions. The RN will have set office hours to be available to residents and families for consultation.

The RN will maintain, track, discharge wound, medication, and behavior alerts. The RN will refer and assess residents in need of outside services (i.e. CBH, Tuality, Home Health, etc.) Conduct Assisted Device assessment for all residents with mobility aides to ensure that they are non-restraining or a managed risk is implemented along with RSC or Administrator. Performing Self-Medication assessments for all residents that are administering their own medications. All members of CCCHD are Mandatory reporters of abuse. Other duties as assigned by and in cooperation with the Administrator.

QUALIFICATION REQUIREMENTS:

Graduate of an accredited program of nursing education. Current registration in this state.

One year experience in an extended care facility or assisted living facility.

Good mental and physical health, sound judgment and high moral standards, and a sincere desire to work with the aged and those with limited capacity for self-care. Patience and compassion are essential.

Ability to manage and supervise employees. Interest and ability in teaching others and assisting them in their skills.

Capacity to develop and learn new theories, techniques and principles and implement them.

Ability to make independent decisions.

Willingness to attend workshops and institutes in field of Geriatrics.

Must function independently, have flexibility and the ability to work effectively with residents, staff, families and support agencies.
