Overview
The Case Manager plays a critical role in supporting individuals and families by assessing their needs, developing personalized plans, and connecting them with appropriate resources. Duties
- Conduct comprehensive assessments of clients’ needs and circumstances.
- Develop individualized service plans that outline goals and necessary interventions.
- Coordinate services across multiple agencies, including Section 8 housing assistance, Medicare resources, and workforce development programs.
- Provide ongoing support and advocacy for clients in areas such as child welfare, addiction counseling, and behavioral health.
- Facilitate access to childcare services and other community resources.
- Monitor client progress and adjust service plans as necessary to ensure effective outcomes.
- Utilize motivational interviewing techniques to empower clients in their journey towards self-sufficiency.
- Maintain accurate documentation of client interactions, progress notes, and service outcomes.
Skills
- Strong knowledge of social work principles and practices.
- Experience with Section 8 housing programs and Medicare systems is highly desirable.
- Familiarity with workforce development initiatives and child welfare policies.
- Background in addiction counseling or behavioral health is a plus.
- Excellent mentoring skills to guide clients through challenges effectively.
