Overview:
We are seeking a highly organized and detail-oriented individual to join our team as a Client Care Coordinator (CCC). As a CCC, you will play a crucial role in providing administrative support and ensuring smooth operations within our clinical outpatient center. This is a full-time position. The CCC acts as a representative of treatment staff and observes all client activity, maintains a visible presence on the clients at the treatment center and at outside activities. The CCC will also circulate throughout the treatment center to alert clients to fire and safety hazards. The CCC is responsible for scheduling transportation to ensure clients arrive safely and on time each day for scheduled therapy groups and activities when needed. The CCC is responsible to:
- Greet and welcome clients/patients in a friendly and professional manner
- Schedule appointments and manage the appointment calendar
- Answer phone calls and respond to inquiries from clients/patients
- Collect and update client/patient information in the system
- Coordinate with medical staff to ensure timely delivery of services
- Maintain confidentiality of client/patient records and information
- Provide general administrative support to the Site Director
Duties:
- Ensuring adherence to the required HIPAA and documentation requirements as identified by county, state, insurance, The Joint Commission, and federal regulatory agencies.
- Assisting the Lab Technician with conducting Urine Analysis (UA) tests and breathalyzer tests on all clients at Clear Steps as scheduled and at random.
- Work closely with the Site Director for administrative and operational needs.
- Follow all Policies and Procedures defined by Clear Steps.
- Assisting with the development of client’s aftercare/discharge plans in collaboration with the Clinical Team to ensure movement to the appropriate levels of care.
- Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, employers, criminal justice agencies, schools, employers, child Protection Services).
- Coordinating ongoing care for client housing and therapeutic needs with the Clear Steps Recovery team, substance abuse treatment providers, healthcare providers, community resources, and family members.
- Documenting all case management and education services, assessments, notes, referrals, and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.
- Obtaining previous records that are relevant to the current treatment episode.
- Overseeing all clients at the facility and at outside activities.
- Setting up transportation for clients to and from sober living and/or home when necessary.
- Utilizing their personal vehicle for picking up lunch for PHP clients, picking up medications at local pharmacies for clients, and doing miscellaneous errands as needed.
- Completing initial intake consent forms, releases of information, and financial agreements with new clients upon admission.
- Orienting new clients to the facility upon admission.
- Attending clinical and administrative meetings to integrate interdepartmental sharing of information.
- Demonstrating adaptability and flexibility to changes in the work environment.
- Complete Incident Reports and communicate any other safety/risk issues immediately to the Site Director.
- Providing a safe, positive, and therapeutic environment for staff and clients.
- Performs other duties as assigned.
Experience:
- High School Diploma or GED required. Bachelor's Degree preferred
- At least 1 year in the field of addiction and/or mental health preferred
- Knowledge of health care, detoxification process, addiction, co-occurring disorders and DSM Criteria preferred
- Previous experience in a clinical outpatient office or similar administrative role is preferred
- Strong computer skills, including proficiency in MS Office Suite
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize tasks effectively
- Attention to detail and accuracy in data entry
