What are the responsibilities and job description for the Recovery Case Manager position at Clear Steps Recovery?
Overview:
The Recovery Case Manager (RCM) is the primary point of contact for client care. They ensure that the therapeutic functions and services are provided through individual case management sessions and group therapy. The RCM meets with all clients upon admission and provides services as needed weekly. The RCM is responsible for facilitating groups scheduled by the Program Director. The RCM is responsible for providing resources, education, and support to individuals, families, and the community, and for promoting healthy lifestyles and healthy choices. Under the Program Director's supervision, and in collaboration with a multidisciplinary treatment team, the RCM ensures post-treatment placement to the most appropriate level of care as recommended for each client. The RCM coordinates care with external providers and community resources such as: Family Members, Primary Care Physicians, Psychiatrists, Outpatient Programs, Residential Programs, Legal Aids, Family Interventionists, University and High School Administrative Staff, and Employee Assistance Program Representatives.
Duties:
- Ensuring adherence to the required HIPAA and documentation requirements as identified by county, state, insurance, The Joint Commission, and federal regulatory agencies.
- Assisting staff with conducting Urine Analysis (UA) tests and breathalyzer tests on all clients at Clear Steps as scheduled and at random.
- Work closely with the Program Director for administrative and operational needs.
- Follow all Policies and Procedures defined by Clear Steps.
- Assisting with the development of client’s aftercare/discharge plans in collaboration with the Clinical Team to ensure movement to the appropriate levels of care.
- Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, employers, criminal justice agencies, schools, employers, child Protection Services).
- Coordinating ongoing care for client housing and therapeutic needs with the Clear Steps Recovery team, substance abuse treatment providers, healthcare providers, community resources, and family members.
- Documenting all case management and education services, assessments, notes, referrals, and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records.
- Obtaining previous records that are relevant to the current treatment episode.
- Overseeing all clients at the facility and at outside activities.
- Setting up transportation for clients to and from sober living and/or home when necessary.
- Completing initial intake consent forms, releases of information, and financial agreements with new clients upon admission.
- Orienting new clients to the facility upon admission.
- Attending clinical and administrative meetings to integrate interdepartmental sharing of information.
- Demonstrating adaptability and flexibility to changes in the work environment.
- Complete Incident Reports and communicate any other safety/risk issues immediately to the Program Director.
- Providing a safe, positive, and therapeutic environment for staff and clients.
- Performs other duties as assigned.
Experience:
- High School Diploma or GED required. Bachelor's Degree preferred
- At least 1 year in the field of addiction and/or mental health preferred
- Knowledge of health care, addiction treatment, co-occurring disorders and DSM Criteria preferred
- Previous experience in a clinical outpatient office or similar administrative role is preferred
- Strong computer skills, including proficiency in MS Office Suite
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize tasks effectively
- Attention to detail and accuracy in data entry
