Job Summary
We are seeking a dedicated and skilled Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in a variety of settings, including sub acute units. This role involves assessing patient needs, developing treatment plans, and collaborating with other healthcare professionals to ensure optimal patient outcomes. The Nurse Practitioner will also have the opportunity to work with diverse populations, including geriatrics, adolescents, pediatrics and individuals with disabilities.
Duties
- Conduct comprehensive health assessments and physical examinations for patients.
- Develop and implement individualized treatment plans based on patient assessments.
- Administer medications and injections as prescribed, ensuring adherence to safety protocols as indicated.
- Provide care for patients in various settings, including outpatient, assisted living, skilled nursing, sub-acute units and other.
- Perform procedures such as wound debridement as needed.
- Educate patients and their families on health management, medication administration, and preventive care.
- Educate facility and unit staff on best practice methods.
- Collaborate with multidisciplinary teams to coordinate patient care effectively.
- Maintain accurate and up-to-date medical records in compliance with healthcare regulations.
- Stay informed about advancements in medical practices relevant to geriatrics, adolescent and pediatric care.
