What you will do :
Support an integrated and comprehensive Performance Improvement program for MacNeal Health Network.
Objectively and systematically assess, monitor, evaluate and improve the processes that affect the quality of patient care, ensuring compliance with : MacNeal Health Network mission and values, The Care Management Program, rules and regulations, MacNeal policies and regulatory standards.
Assess and provide intervention concerning disability resulting from emotional, social and economic stresses of illness.
Identify and provide interventions to meet the psychosocial needs of patients and their families, resource for complex discharge planning needs.
What you will need :
Master's degree required in Social Work.
Licensed Social Worker (LSW) required; LCSW preferred.
Excellent communication skills.
Ability to manage crisis situations calmly and effectively.
Ability to work under stressful conditions and in difficult situations.
Salary Range : $30.00 - $41.59 per hour
Actual compensation will fall within the range but may vary based on factors such as experience, qualifications, education, location, licensure, certification requirements, and comparisons to colleagues in similar roles.
