Hours: Requires a minimum of one 8hr weekend shift every two weeks.

- Objectively and systematically assess, monitor, evaluate and improve the processes that affect the quality of patient care, ensuring compliance with: MacNeal Health Network mission and values, The Care Management Program, rules and regulations, MacNeal policies and regulatory standards.
- Assess and provide intervention concerning disability resulting from emotional, social and economic stresses of illness.
- Identify and provide interventions to meet the psychosocial needs of patients and their families, resource for complex discharge planning needs.
- Master's degree required in Social Work.
- Licensed Social Worker (LSW) required; LCSW preferred.
- Excellent communication skills.
- Ability to manage crisis situations calmly and effectively.
- Ability to work under stressful conditions and in difficult situations.
