We are seeking a Hospice Registered Nurse (RN) Case Manager to provide direct patient care and coordinate with an interdisciplinary team to enhance the care experience for patients and their families.

  • Responsibilities include conducting comprehensive assessments, developing and reevaluating care plans, administering prescribed treatments, and maintaining accurate clinical documentation.
  • Qualifications include a nursing degree, current state licensure, and at least 1 year of nursing experience (3 years preferred), with a background in hospice care being advantageous.
  • We offer comprehensive benefits, including medical, dental, and retirement plans.

Hours are Monday to Friday, 8 AM to 5 PM, with varying on-call requirements.
