Compensation: Approximately $63,085 - $87,325 annually. All individual pay rates are calculated based on the candidate's experience and internal equity.
Overview of Role:
Provide strategic support and oversight of clinical outcome measures, focusing on achieving and maintaining organizational goals while adhering to regulatory and accreditation requirements. By monitoring quality improvement outcomes and initiatives, the CQE works toward minimizing risk, maximizing efficiency, reducing cost, and achieving highest quality outcomes.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Population Health:
- Collaborates with the Director of Process Improvement and Executive Team on execution of the Population Health Strategy for the organization linking strategy to the Medical Home Model of care delivery.
- Establishes and maintains tracking and monitoring systems for clinical quality outcomes, compares outcomes to established goals, regulatory requirements, accreditation standards, procedures, and contractual agreements, and subsequently communicates results to Organization and Clinic Leadership teams.
- Subject matter expert for Uniform Data Set (UDS) requirements.
- Collaborates with the CCHN Quality Improvement team and other CHC Quality peers across the state.
- Serves on committees as assigned.
- Serves as leader and role model to maintain an organization that is patient-centered, committed to patient safety and continuously improving patient healthcare outcomes.
Data:
- Monitors and analyzes data.
- Compares outcomes to benchmarks, established goals, regulatory, and accreditation standards.
- Identifies and communicates best practices and improvement opportunities.
- Prepares internal/external benchmarking data and makes recommendations to facilitate selection of clinical quality improvement goals for the organization.
- Coordinates with Clinical Staff, Business Intelligence Team, EHR support team, and external population health partners, to facilitate the investigation and resolution of clinical quality issues.
- Converts data into useable information.
- Identifies data necessary to establish baseline and on-going measurement of change.
- Data gathering, presentation, interpretation, and analysis including descriptive and inferential statistical analysis when appropriate.
- Performs or coordinates initial and on-going data validation for BI reports and AZARA DRVS reports.
- Azara DRVS (population health/QI software solution) subject matter expert
- Azara/DRVS system administrator for Clinica; adds and removes access to the system for other Clinica staff.
- Maintains data hygiene within Azara DRVS through use of a standard data hygiene calendar.
- Coordinates with CCMCN staff to facilitate investigation and resolution of measure logic issues.
- Creates or facilitates creation of new registries and use cases for the Azara/DRVS system.
- Creates role specific Azara/DRVS training material.
- Trains care team staff in use of the Azara/DRVS system.
- Participates in Clinical Quality/Business Intelligence meetings to offer consultation on new reports, maintenance of reports, and decision support tools.
Quality Improvement:
- Fosters a culture of continuous quality improvement.
- Continuously assesses effectiveness of delivery models, data capture, and resulting clinical measure outcomes.
- Acts as a consultant to Clinical Leadership Teams across all clinics regarding clinical outcome data analysis and quality improvement initiatives.
- Conducts organization wide evaluations to assess standardization of processes that impact clinical outcomes.
- Identifies and shares areas of opportunity based on these evaluations.
- Acts as consultant to develop spread plans for identified organization best practices.
POSITION QUALIFICATIONS:
Education and Experience:
- S./B.A. degree in related field.
- 1-3 years of quality improvement/assurance experience in a healthcare environment.
- Comfortable working with Microsoft office suite; superior skills in Excel spreadsheets.
- Experience with data validation and analysis required.
- 1-3 yrs experience using Azara/DRVS preferred.
Knowledge, Skills and Abilities:
- Communication, research, organization, presentation, and meeting facilitation.
- Holistic approach to problem-solving within a complex system.
- A strong sense of accountability/responsibility to support getting tasks done well; strong organizational skills, detail-oriented, and able to manage competing priorities.
- Knowledge of Quality Improvement tools, methods and strategies.
- Ability to establish/maintain effective relationships with staff at all levels.
- Effective teamwork and coaching skills.
- Familiarity with Electronic Healthcare Record (EHR) navigation.
- Basic skill in building reports from data platforms preferred.
