POSITION SUMMARYThe Community Health & Outreach Coordinator (Coordinator) assists Minnesota residents, in particular the Latine community that resides in the Twin Cities metropolitan area as well as the cities of Austin and Willmar and surrounding areas in Greater Minnesota with health education, vaccination clinics, access to testing, and health recovery services. The Coordinator works to improve access to and promote the health of clients and ensures up-to-date information and guidelines are provided to community members in a culturally and linguistically specific manner. Through outreach, the Coordinator engages, educates, and connects community members with services that address social deterrents of health offered at CLUES.
The Coordinator will have a special focus on COVID Recovery and will coordinate monthly vaccination events, participate in health fairs and community events to promote health and wellness, provide outreach at CLUES Canasta Familiar food distribution events, use social media platforms to educate targeted populations and dispel fears about COVID vaccinations, and leverage other health initiatives in the community.

Schedule : Full-Time. Monday - Friday Office hours, with some evening & weekend hours.
Estimated Pay : $21.25 - $23 per hour (depending on experience & qualifications).

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Community Health Outreach, Education, and Engagement
- Conduct community outreach, engagement, training, and support in connecting community members with resources and services.
- Provide accurate, up-to-date, culturally relevant, linguistically appropriate information related to COVID-19 to priority populations.
- Coordinate, schedule, and facilitate COVID vaccination clinics in partnership with health care providers.
- Address vaccine hesitancy and challenges navigating community resources.
- Distribute at home test kits at vaccine events and other community resource events to promote testing and reduce spread.
- Support community members in navigating and accessing resources such as housing, employment, SNAP enrollment, and health insurance navigation.
- Organize and participate in community events, workshops, and meetings to promote organizational programs, share information, educate, and connect individuals and families with services.
- Create and implement outreach plans that align with the organization's goals and objectives.
- Connect with community-based organizations monthly to share information, resources, and reach additional community members. Maintain a database of community contacts, partners, and stakeholders.
- Conduct community outreach, flyer distribution, social media outreach, and partnerships with community programs and organizations.
- Utilize various communication channels (social media, email, newsletters, etc.) to reach and engage target audiences.
- Collect and prepare reports on outreach activities and outcomes for internal and external stakeholders.
