West TN SOAR CASE MANAGER / Liaison
Salary: Negotiable
Reports to: Regional SOAR Coordinator
Status: This is a full-time position that is funded 100% by a state grant
Duties and Responsibilities
- The West TN SOAR Case Manager will prepare and submit SSI/SSDI new applications to Social Security Administration on behalf of service recipients who have been hospitalized for long periods before discharge.
- The SOAR Case Manager position will also assist patients preparing for discharge whose SSI/SSDI benefits have been previously approved for SSI/SSDI, but stopped due to extended hospitalizations. The SOAR Case Manager will advocate and work to reinstate benefits and restart payments upon discharge.
- The SOAR Case Manager will serve as the primary liaison for Social Service staff at Western Mental Health Institute and SSA regarding SSI/SSDI applications being prepared and submitted.
- The SOAR Case Manager will primarily be located at Western Mental Health Institutes to serve the targeted population. Office space will be provided at the mental health institute.
- SOAR Case Manager will be required to work 25-40 hours a week at Western Mental Health Institute and CMI Healthcare Services. The SOAR Care Manager is projected to spend two to three days per week at Western Mental Health Institute and one day per week at CMI Healthcare Services.
- The SOAR Case Manager will be required to participate in monthly state oversight meetings to discuss progress and outcomes, as well as routine meetings with the SOAR Coordinator and Director of Special Programs and Services to ensure we are on task of meeting goals and expectations of the position.
- Other duties as assigned
Minimum Requirements
■ Minimum BA/BS required; Master’s degree preferred
■ Experience providing direct mental health services to consumers
■ Excellent organizational skills and time management skills
■ Excellent oral and written communication skills are required
■ Familiarity with the SSI/SSDI process would be helpful but not required
■ Good computer skills
