Compensation: $85,000 - $95,000 Annual Salary

Position Description:
We are looking for a dedicatedMDS Coordinator for our Skilled Nursing Facility ( SNF ) in Stow, Ohio. This is a full-time, in-person position, Monday - Friday 9am - 5pm
Qualifications of the MDS Coordinator:
Valid Registered Nursing ( RN ) license in Ohio.
2 years as an Assistant Director of Nursing ( ADON ) at a Skilled Nursing facility ( SNF ) with experience in leadership and nursing staff development.
2 years of experience as an MDS Coordinator within a Skilled Nursing Facility ( SNF ).
Clear understanding of procedures and patient care within a SNF setting.
General understanding of budgeting and financial reporting.
Responsibilities of the MDS Coordinator:
As the Assistant Director of Nursing ( ADON ) for our Skilled Nursing Facility ( SNF ) you will lead and mentor all nursing personnel..
Perform evaluation of patients using the MDS assessment tool, covering areas like physical function, cognitive abilities, medical conditions, and psychosocial needs.
Utilize data from MDS assessments to create individualized care plans with specific goals and interventions for each patient.
Enforce policies for high quality standards of patient care.
Direct budgets and monitor expenditures.
