Weekend Home Health Aid

Job Summary
We are seeking a compassionate and dedicated Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient-centered care, as well as the ability to work independently while collaborating with a multidisciplinary team.

Duties

• Conduct comprehensive patient assessments, including vital signs and patient observation.
• Develop and implement individualized care plans tailored to each patient's needs.
• Provide essential nursing services such as catheter care, memory care, and assistance with daily living activities.
• Administer medications and treatments as prescribed by physicians.
• Educate patients and their families on health management, including dementia care strategies.
• Maintain accurate and timely documentation in compliance with HIPAA regulations.
• Collaborate with healthcare professionals to coordinate patient care effectively.
• Support patients with disabilities by providing necessary assistance and ensuring their safety.

Skills

• Proficient in memory care techniques and dementia care practices.
• Strong ability to assist patients with daily activities and personal hygiene tasks, including laundering when necessary.
• Knowledgeable in catheter care procedures and other nursing interventions.
• Excellent observational skills for monitoring patient conditions and identifying changes.
• Familiarity with HIPAA regulations to ensure patient confidentiality and data protection.
• Compassionate approach towards working with individuals with disabilities, fostering a supportive environment.
