Job Overview:
As a Community Outreach Coordinator, you will play a key role in supporting various business functions, including Account Management, Recruiting, and Scheduling/Operations. Your ability to multitask, communicate effectively and proactively, while being detail-oriented will ensure an excellent client and Caregiver experience.
Key Responsibilities
Process inbound client referrals via email, software portals, faxes, and phone calls promptly and accurately.
Create and maintain detailed client profiles, ensuring all information is up to date.
Handle insurance authorization requests in WellSky or related software systems.
Manage and update tasks in WellSky, in real time, to ensure accurate tracking of client referrals, authorizations, and statuses, ensuring timely follow-up
Coordinate with family caregivers to initiate the hiring process and collaborate with the recruiting team.
Develop care plans and care schedules upon client referral authorization approval, entering related details into scheduling software
Communicate care start dates with referral sources and relevant stakeholders.
Collaborate with healthcare providers and internal teams to ensure excellent service delivery
Qualifications:
1-3 years of experience in administrative roles, preferably in healthcare or community outreach.
Experience with WellSky or similar software is highly preferred.
Proficiency in Google Suite (Docs, Sheets) and other business software.
Strong organizational, communication, and problem-solving skills.
Ability to adapt to shifting priorities in a fast-paced environment.
Collaborative team player who can also work independently.
