Position Title: Bridge Program Therapist
Location: Coastal Connecticut Counseling - Milford, CT
Reports To: Clinical Supervisor

Position Summary:

Overview:
The Bridge Program is designed to provide comprehensive care and support for individuals in transition between different levels of mental health care. Specifically, the program serves individuals who are waiting for admission to a higher level of care (such as inpatient treatment or intensive outpatient programs) or those stepping down from a higher level of care and requiring continued support in a lower-intensity setting. The Bridge Program ensures that individuals receive timely, personalized interventions to maintain stability, promote well-being, and support their mental health journey during these critical periods of transition.

Target Population:
The program serves individuals who are experiencing a range of mental health challenges, including mood disorders (e.g., depression, bipolar disorder), anxiety disorders, psychotic disorders, and trauma-related conditions. The program is designed for:

  • Individuals waiting for admission to a higher level of care, such as inpatient psychiatric hospitalization, partial hospitalization programs (PHP), or intensive outpatient programs (IOP).
  • Individuals transitioning from a higher level of care (e.g., inpatient care) who need ongoing support to reintegrate into the community or step down to a less intensive treatment setting.

Program Objectives:

  • Provide Stabilizing Support: To offer structured, individualized support that helps individuals maintain or improve their mental health while they await higher-level care or transition to a lower level of care.
  • Bridge the Gap: To reduce the risk of relapse or deterioration by offering targeted interventions that prevent individuals from falling through the cracks during transitional periods.
  • Enhance Coping Skills: To teach and reinforce coping strategies, emotional regulation techniques, and self-care practices to manage mental health symptoms effectively.
  • Promote Connection: To ensure individuals have access to therapeutic support, peer networks, and community resources during their wait or transition.
  • Facilitate Long-Term Recovery: To equip individuals with the tools and resources they need to move forward in their mental health journey, whether that means stepping up to a higher level of care or stepping down to a lower level.

Core Services:

  • Individualized Care Plans: Participants work with a dedicated mental health clinician to develop a personalized care plan based on their unique needs, treatment goals, and current mental health status.
  • Therapeutic Support: Regular therapy sessions, using evidence-based approaches such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), and Motivational Interviewing (MI), tailored to each individual's needs and progress.
  • Group Therapy: Facilitated group therapy sessions designed to foster peer support, enhance social engagement, and teach new coping skills in a safe and supportive environment.
  • Case Management: Work closely with participants and their families to help them navigate and access additional resources for stability during the transition period.

Qualifications:

  • Education: Master’s Degree in Social Work, Counseling, or Marriage and Family Therapy or a related field from an accredited institution.
  • Licensure: Current licensure as a therapist (LCSW, LMFT, LPC, LMSW, LMFT-A or LPC-A) in the state of Connecticut.
  • Experience: Minimum of 1 year of experience providing therapeutic services preferably with experience in crisis services.
  • Skills:
  • Strong clinical assessment and diagnostic skills.
  • Proficiency in using evidence-based therapeutic techniques.
  • Excellent communication and interpersonal skills.
  • Ability to work collaboratively within a multidisciplinary team.
  • Proficiency in using electronic health records (EHR) systems.
  • Comfort in navigating crisis situations and assessment
