What are the responsibilities and job description for the Transitional Living Program Case Manager position at Coastal Horizons?
The position of Open House TLP case manager involves participation in the screening, admission, orientation, transition planning, and discharge process into or out of the Open House Transitional Living Program. A case manager needs to assess a client’s life domain areas, provide local resources for the client to meet those needs, and follow up to assure a client has made the necessary appointments. The case manager will arrange for clients to receive services identified in the client’s Individualized Service Plan. The case manager will provide transportation for clients to and from appointments, community activities or other events when clients are unable to find transportation themselves. The case manager works closely with the client and service providers to assure appropriate transition plans upon discharge from the program.

- Graduation from an accredited four-year college or university in human services or related field
- At least two (2) years related work experience with homeless or vulnerable youth
- An equivalent combination education and experience.
