The Intake Supervisor plays a crucial role in ensuring the efficient processing of patient information and documentation within a home healthcare setting. This position is responsible for overseeing the intake process, managing patient service inquiries, and ensuring that all documentation is accurate and compliant with medical standards.
Responsibilities Include:
- Oversee the intake process for new patients, ensuring all necessary documentation is collected and reviewed.
- Conduct documentation reviews to ensure compliance with healthcare regulations and standards.
- Conduct thorough assessments of prospective patients to determine their care needs and eligibility for admission, collaborating with healthcare professionals as needed
- Assign initial ICD 10 code to initiate authorization process for all referrals for HMO patients
- Establishing & maintaining relationships with local hospitals, physicians & community organizations for the purpose of generating consistent admissions to the facility
- Understand the admissions process including policy, procedures, assessments & paperwork requirements
- Communicate special needs of new admissions to staff to ensure a smooth transition
- Work closely with the CHHA's interdisciplinary team, including nursing staff, social workers, and administrators, to coordinate admissions and provide comprehensive care to patients
Position Requirements:
- Degree in Nursing, Social Work or a related field
- Licensed RN or Social Worker strongly preferred
- Previous experience in a healthcare admissions or discharge environment
- Experience working in a CHHA (Certified Home Health Agency)
- Previous experience in a management, supervisory or team lead role
- Knowledgeable in Home Care billing (Medicare, Medicaid, 3rd party payor)
- Knowledgeable in ICD-10 medical coding

Salary : $90,000 - $110,000
