The Oasis Reviewer and Documentation Specialist plays a crucial role in ensuring the accurate coding and review of nursing assessments (SOC/ROC/Recert) and 485 data in accordance with Centers of Medicare and Medicaid (CMS) and Department of Health regulations.

Key Tasks Include:
- Perform comprehensive home health documentation reviews within multiple EMR systems; reviews will focus on coding and OASIS accuracy in accordance with regulatory requirements
- Review clinical OASIS (SOC/ROC/Recert assessments) and 485 data to ensure accuracy, appropriateness, congruency, completeness, and compliance with federal and state regulations and client policy
- Ensure appropriate ICD-10 coding and sequencing as it relates to the patient's medical condition
- Perform final review and lock OASIS
- Communication with service offices monthly and as appropriate with a focus on documentation trends, star ratings and potential revenue impact
- Prevent or decrease the occasion of Medicare denials by assuring proper coding on the plan of care and accurate OASIS documentation
- Act as a resource for clinicians as it relates to OASIS, coding, and PDGM guidelines
- Communication with service offices monthly and as appropriate with a focus on documentation trends, star ratings and potential revenue impact.
- Work closely with the CHHA's interdisciplinary team, including nursing staff, social workers, and administrators, to coordinate admissions and provide comprehensive care to patients
- Maintain knowledge of regulations governing Medicare/Medicaid/Managed Care reimbursement processes
Position Requirements Include:
- Graduate of an accredited school of nursing
- Licensed as an RN or LPN in the State of New York
- HCS-D pr BCHH-C Home Health Care Coding certification required
- OASIS (COS-C/HCS-O/COQS) certification required SHP experience a plus!
- 2 years of recent home health coding and OASIS review experience
- Background in a management, operations, or quality assurance role within a home health agency preferred
- Extensive knowledge of CMS home health regulations including OASIS, coding and plan of care requirements
- Advanced computer skills with experience with one or more EMR systems
- Must have a designated workspace free from noise and distraction
Salary : $75,000 - $90,000
