Cogir Senior Living is hiring an experienced, diligent, and compassionate Resident Care Coordinator (LVN) for our beautiful retirement community, Cogir of Sonoma Plaza (formerly Cogir on Napa Road).
The Resident Care Coordinator liaises between residents, families, and staff. They are responsible for the overall implementation, delivery, and coordination of resident care in the community. They will support the Health and Wellness Director by assisting in scheduling, managing staff, and ensuring that systems are in place to provide quality assisted living services in all resident care areas. Duties include problem-solving resident concerns and coordinating care with the caregiver team and external partners.
