Social Worker
Job Description:
- Assists clients/families in adjusting to and decreasing symptoms of mental challenges following the treatment plan.
- Provides individual, family, and group counseling to adults and adolescents when needed, especially to clients with substance use problems.
- Conduct weekly groups, leading client sessions utilizing accepted therapy models, group therapy techniques, and basic skills training.
- Interviews families, support persons, and patients to gather pertinent information for assessment completion.
- Acts as a liaison between Cognitive and LCSW as needed.
- Analyzes patient data and prioritizes patient needs accordingly.
- Manages the care of patients during their attendance in the program and the discharge process.
- Updates and/or revises care plans as patient needs or conditions change. Reports significant changes to the Psychiatrist and Supervisor (as needed).
- Performs all aspects of patient care in an environment that optimizes patient safety, reducing the likelihood of medical/medication/health care errors.
- Collaborates with patients and all involved parties to develop an individualized patient care plan.
- Performs crisis intervention counseling if necessary.
- As a patient advocate, ensures patient’s psychosocial needs are met.
- Maintains patient confidentiality as required per HIPAA guidelines.
- Determines causal relationships between substance addiction, socio-economic factors, and personality traits.
- Works cooperatively with community agencies in the coordination of rehabilitation services for clients.
- Counsels family members of clients to provide emotional support and information.
- Participates in the evaluation of counseling methods and documents client progress in charts.
- Assists in coordinating and synchronizing substance abuse and service plan issues.
- Supports consumers’ steps towards Recovery and Wellness.
- Creates an organizational culture that respects and celebrates the diversity of our consumers.
- Assists with the Psychiatric Comprehensive Assessment.
- Conducts one-on-one sessions with clients.
- Performs additional duties as required.
Requirements:
- Social Worker must have a minimum of 2 years of experience.
Job Type: Full-time
Pay: $25.00 and up (depending on qualifications)
