Position Compensation
The Adult Case Manager position is currently a per diem position with competitive pay based on experience and performance. Current rates range from $25.00 - $35.00 hourly. At this time Cognitive Community Care, LLC does not offer traditional benefits however, we do comply with State of Maine Department of Labor offering required paid time off including but not limited to, Earned Benefit Time to be eared at 1 hour per 40 hours worked.
Position Summary
As a Case Manager, you will have a high degree of flexibility with control over your own schedule as a per-diem employee. We require a minimum of 25 billable hours weekly, new employees will have 60 days to meet the billable requirment. You will provide direct support to individuals experiencing long-term mental health challenges. You will assess client needs, develop and implement service plans, and collaborate with community resources to ensure individuals receive the comprehensive support necessary for their well-being. This role requires strong communication skills, clinical insight, and the ability to work both independently and as part of a team.
Key Responsibilities
- Conduct client intakes via telehealth or in-person, depending on client needs.
- Develop individualized treatment/service plans in collaboration with clients.
- Ensure clients receive appropriate services and support in compliance with state regulations and MaineCare policies.
- Maintain timely and accurate case documentation, including service notes, collateral contacts, and required reports.
- Provide ongoing case management support, monitor progress, and adjust service plans as needed to align with client goals.
- Engage in regular supervision meetings to enhance professional development and case consultation.
- Act as a liaison between clients and community resources, including housing, social services, and healthcare providers.
- Uphold confidentiality and professional ethics in all client interactions.
- Perform other related duties as assigned by the supervisor.
Qualifications & Skills
- Bachelor’s or Associate’s degree in a relevant human services field preferred.
- At least one year of experience working in mental health (preferred).
- Active MHRT-C certification or eligibility for provisional MHRT-C.
- Strong ability to conduct client assessments, develop treatment plans, and coordinate services.
- Knowledge of community resources and service systems (housing, social welfare, education, etc.).
- Excellent written and verbal communication skills.
- Ability to work both independently and collaboratively within a team.
- High level of professionalism, ethical integrity, and confidentiality.
- Proficiency in clinical documentation and understanding of regulatory requirements.
