Per Diem Case Manager
Job Type: Per Diem, with Full-time and Part-time options available
Weekly Hours: 20-40 hours per week
Pay: $31.50/hour for billable hours, $15.00/hour for documentation, meetings, and travel

Who We’re Looking For:
- Education & Certification: Bachelor’s degree in Social Work or a related field. Must have or be able to attain MHRT-C certification within a 90-day probationary period.
- License: Valid driver’s license in good standing.
- Passion for Community Service: Our case managers support clients in finding essential resources and achieving independence in all areas of life.

Your Role:
- Conduct initial assessments to understand the needs and strengths of each client
- Develop individualized treatment plans and work hand-in-hand with clients and families to achieve set goals
- Advocate and connect clients to essential services: mental health, medical care, education, housing, and more
- Collaborate with providers to ensure effective support and crisis management
